Pharmaceutical care faces great challenges due to ever increasing drug costs, increasing numbers of adverse events, poor adherence, medication errors and inadequate communication across the primary/secondary care interface. The recent global economic crisis has resulted in increased awareness of the costs and outcomes of pharmaceutical care.
